How to Create a Sign-up Form

Create online forms for trials, memberships, events, belt tests and more, then share the link or QR code so families can sign up and pay on their own.

Written by Ben Ryoo

Forms let families sign up online for a trial, a membership, an event, a belt test or a product, without coming to the front desk. Each form has its own link and QR code you can share anywhere.

Create a form

Step 1: Go to Marketing Tools → Forms and click Add new form.

Step 2: Choose a form template and click Continue.

Step 3: Enter a Form Title and an optional description, then click Continue.

Step 4: Set up the form in the editor (see below), click Preview to see it the way families will, then click Save.

Set up the form

The editor has three parts: Form Option, Payment Methods and Add Fields. The preview on the right updates as you make changes.

Form Option

Option

What it does

Allow Multiple Participants

Lets one parent register more than one student, such as siblings, in a single form.

Family Membership

Charges once per family instead of per person. All family members under the membership share a single payment.

Custom Landing Page

Sets the page people see after they submit the form. For a trial form, you can send them straight to your trial classes.

Add Tags to Participants

Adds tags to everyone who signs up, so you can find and filter them later.

Email validation

Asks people to confirm their email with a 6-digit code. We recommend keeping it on. See Why We Recommend Email Verification on Your Forms.

Payment Methods

Choose how people can pay: Credit Card, ACH or Cash.

  • ACH lets families pay from a bank account, including new families signing up for the first time. Your school needs bank transfers set up first. See Accept Bank Account (ACH) Payments.

  • A method you've turned off for that revenue type in your payment settings can't be used on the form.

  • If you don't take payments online, turn on Cash only.

Add Fields

  • Header: the title, description and header image families see at the top. You can change or remove the image.

  • Participant Information (Student): name, email and phone number are always included. Add Date of birth, Gender, Address or your own Custom question.

  • Registration: check the memberships (or trial programs) people can choose from. They appear in the same order as on your Membership Plans page.

  • Payment, Buyer Information and Agreement: added automatically when the form takes payment. Buyer Information is for the parent or guardian. If the membership has Terms and Conditions turned on, the agreement and signature appear at the end of the form.

Share your form

  1. Go to Marketing Tools → Forms and open the form.

  2. Click Share.

  3. Click Copy Link, or Download the QR code.

Put the link on your website and social media, or in emails and texts. Print the QR code on flyers or display it at your front desk so anyone can scan and sign up.

Manage your forms

Open a form to see its settings and submissions, including form views, total participants, total payments, and who referred each new student (Referred by). Use the Actions menu to:

  • Edit: change the form. The link stays the same.

  • Copy: make a new form with the same settings, fields and memberships. "- Copy" is added to the title. It's the fastest way to make a similar form.

  • Set to Inactive: take the form offline without deleting it.

  • Delete: remove the form.

Common questions

Can I change a form after I've shared it? Yes. Edit the form and save. The link and QR code stay the same.

Can new families pay by ACH? Yes. Turn on ACH under Payment Methods, and families can enter their bank account when they sign up.

Families who pay by bank account don't see ACH on the form. Why? ACH is turned off on that form. Edit the form, turn on ACH under Payment Methods, and save.

A payment method is turned off and I can't turn it on. Why? It's turned off for that revenue type in your payment settings. Allow it there first.

I need a separate form for each membership plan. Is there a shortcut? Yes. Create one form, then use Actions → Copy and change only what's different.
